Following 07 States fall under the Consular jurisdiction of the CGI Chicago:
Illinois, Indiana, Iowa, Michigan, Minnesota, Missouri, and Wisconsin
Applicants from these states only may apply at the Consulate General of India, Chicago. Please apply strictly as per your jurisdiction for which you have verifiable proof of address.
Please note that no Commercial Documents are attested by the Consulate. The same should be apostilled from US Government Authorities for presenting in India.
Apostille Convention: The Hague Convention, to which India and USA are signatories, abolishes the requirement of legalization for Foreign Public Documents. With the certification by the Hague Convention 'Apostille', the document is entitled to recognition in India and vice versa and no certification by the Consulate of India is required. The Convention applies to public documents which have been executed in the territory of one Contracting State and which have to be produced in the territory of another Contracting State.
The Consulate does not certify the genuineness of any document issued by authorities in India. If any document issued by authorities in India (such as Birth and Death Certificates, Affidavits, Educational Documents / Diplomas, Marriage Certificate and Driving License, etc.) are required to be presented to authorities in the USA, these should be apostilled in India. It is not mandatory that the apostilled documents should be attested by the Consulate officials before presenting it to authorities in India.

CGI, Chicago has no agents for rendering these services and applicants.
The Consulate reserves the right to reject attestation of documents, the contents of which are objectionable or contrary to the Rules.
GENERAL INFORMATION
Local documents like Bank Statement, Salary Statement, Employment Letter of the Applicant, Driving License, etc. need to be Notarized and uploaded by the applicant and only the signature of the applicant will be attested. On each document be attested, applicant will mention “I TAKE FULL RESPONSIBILITY FOR THE CONTENTS OF THE DOCUMENT”. After that the applicant will put his full name and signature (as per passport) on each document submitted for attestation.
Note:- It is mandatory for the documents issued by the US Government authorities to be apostilled by the relevant authorities in the USA before presenting the document for attestation at the Consulate.
B. For attestation of Indian passport, Driving License, PAN Card, Adhar Card, Voter ID, Educational documents etc., applicant may submit Notarized copy of the same. Documents which are not apostilled from India will not be authenticated by Consulate. Such documents will only be attested as ‘True Copy Attestation Only”. On each document to be attested, applicant will state that “I TAKE FULL RESPONSIBILITY FOR THE CONTENTS OF THE DOCUMENT” and write full name and sign, as in the passport.

I-797, I-140 or I-20 (If approval is pending, attach a handwritten note detailing the efforts being taken to regularize status)

How to apply?
Apply for Misc. Service through eCCSS Portal only.
It is mandatory to register the applicant on the above mentioned eCCSS portal before in-person visit or sending the applications through mail to the Consulate.
Once you are ready to apply for Misc. Service, please click on above mentioned “Apply for Misc. Service through eCCSS Portal” tab and the system will redirect you to eCCSS portal.
Please check the required documents, fees, etc. for particular miscellaneous service without registering on eCCSS portal. Once you are sure that you would like to avail particular misc. service please follow the below steps.
Signup and create your account using the New User Tab.
Activate your account using OTP received on your registered email ID.
Please login into your account and apply for the required service.
Users can also avail multiple services with the same account.
After selection of the service, please upload the necessary documents on eCCSS portal.
On successful submission of the documents, an electronic acknowledgment will be generated, which the applicant can print after logging in to his/her account.
After scrutiny of the documents by the Consulate, the applicant will receive confirmation for payment of fees, which can be seen in the dashboard of his/her account.
On receipt of confirmation for payment of fees, the applicant is required to pay the required fees
Fees: When applying by mail specified fees must be paid only as Money order / Cashier's Check drawn in favour of Consulate General of India, Chicago. All fees specified and the payment of additional fee of $ 2.00 (Two Only) towards Indian Community Welfare Fund (ICWF) must be drawn as a separate Money order / Cashier's Check. Payments of all specified fee and fee towards ICWF when made must be either as Money Order / Cashier's Check drawn in favour of Consulate General of India, Chicago. ALL FEES ARE NON-REFUNDABLE.
CASH, PERSONAL CHEQUES, CREDIT CARDS, DEBIT CARDS OR OTHER BANKING INSTRUMENTS ARE NOT ACCEPTED.

How to send Physical copy of the application to Consulate?
The applicant is required to send a physical copy of the submitted application form, fees in form of Cashier’s Cheque / Money Order / Demand Draft (whichever is applicable), copies of documents OR notarized copies of the documents and a return mail envelope to the Consulate by postal mail to the Consulate address as mentioned above.
The notarized copies of the documents will be matched and verified with the uploaded documents. On the verification of the same, misc. consular service will be provided to the applicant.
